The Report Form is the complete run down of the accession ordered on the patient.

The Header Section:
At the top of the Report Form is the header. On the left hand side has the logo and name of the laboratory and below it the name of the Client. On the right hand side, the patient demographic is listed; Name, Physician, I.D., DOB, Age, Sex, Accession, and status of of the test.
The Body Section:
The Body Section is broken down into 5 columns; Test, Normal, Abnormal, Units, References. Test is the name of the test that was run on that patient. If the result falls into the "Normal" section, that means that the result was within the reference range for normal patients. If the result falls into the abnormal column, it will have a L or H next to the number indicating if the result is abnormally Low, or abnormally High. The units are. The Reference is the normal ranges that the results of the test should be.
You may notice a "prior" links next to the reference columns. If you click on this you will get the "Prior Graph".
How to print:
When you access the Patient Folder, it will pop up in your standard browser. All you have to do is go to File -> Print, or use the Print shortcut located on your navigation bar.
